hold action until all possible causes were at least written down.
The segments of the diagram — maintenance, personnel,
systems and material — are only four of the possible areas to
assess. In special situations, there may be others. Consider all
the possibilities with a fishbone diagram and use the Kepner-
Tregoe method to help narrow down the list. Finally, always
remember to look for something that has changed recently. This
will be the best approach to determine the cause of your next
equipment failure.
