2 WE RAISE HEALTHY CHILDREN O ur charity serves more than 114 individuals each week through our therapeutic riding program. Our team of hard-working horses provide equine-assisted activities to a diverse group of riders each week. Our programs offer support to individuals with disabilities such as Down syndrome, autism, spina bifida, multiple sclerosis, traumatic brain injury, cerebral palsy, amputation, mental and physical disabilities, attention deficit disorder and more. We offer a School Partnership Program to benefit special needs students from area elementary and middle school systems. Students participate in riding and theory classes. We partner with special education teachers to reinforce lesson plans and enhance the life skills and development of special needs students. Completing the Paper Pledge Form Use this example pledge form as a guide if you have any questions about filling out a paper pledge form. The Donor MUST keep DONOR COPY for tax purposes. Return CFCSHR OFFICE COPY to the CFC Office. Submit the PAYROLL / PSD COPY to your agency's Payroll/PSD office. If you are unsure of where to turn in your pledge form, or who your CFC Coordinator/Key Worker is for your federal agency/command, please contact your CFCSHR Representative, Kenyetta Fauntleroy at (757)853-8500 ext.155 or by email at kfauntleroy@ unitedwayshr.org. If you are having trouble finding a charity or cause that you would like to donate to, please visit us at www.cfcshr.org and click on "SEARCH CHARITIES." If your specific charity is not listed, then it was not approved for our South Hampton Roads region or the Universal Giving Program. Top 5 Common Errors 1. Illegible. If it is unreadable, the pledge may not be processed correctly. 2. Incorrect Amount Listed. Designate gift amount $ X per pay period (24 or 26pp). Enter specified amount, of total gift, to each designated charity. 3. Invalid Designation. Pledges can only be made to charities listed in this region's brochure/charity lists. 4. Social Security Number/Employee ID#. Payroll deduction cannot be authorized/ processed without donors' SSN/EIN. Payroll offices REQUIRE your SSN/EIN, not CFC. 5. Signature. Only sign form if contributing via Payroll Deduction. Payroll deduction cannot be authorized/processed without signature. September 2016 Dear Military and Federal Family of South Hampton Roads: The Combined Federal Campaign (CFC) mission is to support and promote philanthropy through a voluntary program that is employee-focused, cost-efficient, and effective in providing all federal employees the opportunity to improve the quality of life for all. As we commence the 2016 Combined Federal Campaign, we can look back with satisfaction at the results of our hard work in 2015. Last year, our military and federal personnel donated $3,113,833 to the CFC of South Hampton Roads. Of this amount, $802,901 was donated to local area charities, making a direct, positive impact on those in our own communities. This is how we gave back and I want to thank all who contributed in 2015. I am an avid supporter of CFC and believe we all should make a difference through our voluntary contributions. This is OUR campaign and WE have the opportunity to assist children, the homeless, and others who are in need. Additionally, we are able to support foundations for the arts, the environment, medical research, animal protection, adoption services, and many other deserving causes. The CFC is a designation-based campaign and you are able to decide exactly what charity your money supports and in what percentage. If you choose not to designate, the funds will be distributed proportionally to those charities in our campaign who receive designated donations. These are your dollars supporting those charities that you know and trust. We want to ensure your support is given appropriately. There are many worthwhile charities looking for support and I encourage you to make use of the 2016 CFC charity list when considering your donations, to find a charity worthy of your support. The charity list is available in PDF format and in a word-search version on the CFC Website, www.cfcshr.org. Federal employees can now give to any local CFC approved charity, through the Universal Giving Program, regardless of where the charity is located. This excel database lists thousands of additional approved charities and can be found on the CFC Website also. For those who choose to donate online, NEXUS is the online pledge system, which can also be accessed from the website. Please take advantage of this easy and convenient tool. The Office of Personnel Management's (OPM) CFC Operations Office has assigned a unique five (5) digit code number for each charity. However, the charity you want to donate to may not be a part of the 2016 CFC campaign, so please check to ensure that they are participating. We highly encourage you to donate using payroll deduction. By spreading out your gift over the year, a small donation per pay period can build to a larger sum total with overall significant impact. It only takes a few minutes to donate online or to fill out the pledge form. Your contributions will go far in helping out those less fortunate. They may even have impacted someone you know or even you! If you have previously given, thank you for your support. If you have not given before, we ask that you consider voluntarily contributing by joining the tens of thousands of military and federal employees in South Hampton Roads who have made a contribution and helped someone in need.
