PARTICIPATION WAIVER
By enrolling in any and all PRC/MAA programs, I hereby agree to assume all the risks of
any & all injury to, disability or death of the participant associated with the program.
I agree to release the PRC/MAA, City of Parsons, USD #503 School Dist., their employees,
agents, representatives, coaches, and volunteers from any liability resulting from any
ordinary negligence that may arise in connection with the program. I also understand
that during my or my child's participation in PRC/MAA programs, my photograph and/or
the photograph of my child might be taken and I agree that my photograph and/or my
child's photograph, including video photography or any other reproduction of my
likeness or the likeness of my child, may be used without charge by the program
sponsors, producers, organizers, and/or its assigns for such purpose as they deem
appropriate.
