24 Covetrus | ph: 855.724.3461 | fx: 888.329.3861 | northamerica.covetrus.com DEA Form 222's As of October 30, 2019 the DEA implemented a new single-sheet format for the DEA Form 222. Many of you are already using the single-sheet DEA Form 222. There is a two-year transition period which will allows the existing triplicate version of the form to continue to be used, but that will end October 30, 2021. The single-sheet 222 order forms are still ordered through the DEA website at www.deadiversion.usdoj.gov/. The purchaser completes the order in the same manner as before. The purchaser must make a copy of the single-sheet 222 Form before submitting the form to the supplier. A copy of the forms are to be kept for a period of at least two years or more if your state requires a longer record retention period. The supplier will receive the 222 order and processes as normal. Upon receipt of the schedule 2/2N controlled substance the purchaser must indicate the date and amount of schedule 2/2N controlled substances received on the copy of the 222 order form and keep the completed copy as a record. The supplier will keep the original 222 order for their own records. How does a practitioner record a breakage or spillage? If the breakage or spillage is not recoverable, the incident should be recorded in your controlled substance records and have two people that witnessed the incident sign the records. No other forms need to be filled out. This is not considered a loss of product or destruction of product. If the breakage or spillage is recoverable, the controlled substance must be disposed of according to the DEA regulations, preferable through a reverse distributor so they can supply you with the appropriate records. If you chose to destroy on your own, a DEA Form 41 must be completed. Please keep in mind that this is very high level when it comes to controlled substance recordkeeping. We have barely scratched the surface. Not only should you be keeping good records, but making sure it is not just up to one person. Job sharing can help prevent diversion.
