individuals into the exchanges, but the lack of coordination money is appalling. Meanwhile, the rates for young males insurance groups are rising at 35 percent. Businesses that have more than 50 employees must offer health care coverage in 2014, and an employee over the age of 26 who is working will be assessed a tax for not having health care coverage beginning in 2015. Many associations and industries are working on solutions to rising health care costs with the most promising being a combination of employer paid health benefits, employee paid wellness benefits and the migration of hospitals from provider of benefits to the coordinator of benefits. In the early 1980s, doctors who worked for HMOs were called gatekeepers. Now they are called health care facilitators. Doctors, nurse practitioners, health care coaches and employees will all be put together in a system in which correctly coordinated benefits will work to benefit everyone. The employees will see zero or near zero co-payments, the doctors will see higher reimbursements based on better heath care outcomes for their patients and, finally, the hospitals will see a better bottom line because they will reduce their reliance on health insurance companies and their profit structures in providing care to patients by partnering with insurance brokers. In the past, brokers found customers for the insurance carriers. They now will find patients for the hospitals, which will leave the insurance carriers with a smaller share of the heath care pie. The next step will be employer-sponsored clinics to treat their employees onsite. This will lead to the next great health care experiment: studying the economics of fewer employee days out for medical purposes.
