Host Modulating For
Periodontal Treatment
The conventional treatment for
periodontitis has focused on
mechanical removal of bacterial
agents, thereby reducing the
infectious challenge and leading to
resolution of inflammation and control
of periodontal disease.
It is now understood that the primary
etiologic factor for periodontal
disease is bacteria, but they are
insufficient to cause the disease alone.
The excessive host inflammatory
response and inadequate resolution
of inflammation is critical to the
pathogenesis of periodontitis. The
inflammatory response (which
is protective initially) if chronic,
is responsible for much of the
destruction of the hard and soft
periodontal tissues. The purpose of
host modulation therapy is to restore
the balance of proinflammatory
(destructive) mediators and anti-
inflammatory (protective) mediators in
periodontal health.
Modulation of host response is now
considered a novel promising therapy
and consists in modulating the host
defense mechanism in response to
inflammation. Chronic inflammation
supports the growth of pathogenic
bacteria through the production
of tissue breakdown products.
Resolution of inflammation effectively
eliminates the pathogen from the
lesion by removing its food source.
The purpose of host modulation therapy is
to restore the balance of proinflammatory
(destructive) mediators and anti-inflammatory
(protective) mediators in periodontal health
