15 Q3 2022 | Animal Health Solutions New Host Modulating Products There are many new host modulating products that are currently in use in humans and animals in the research phase. There are very few that are safe, economical and recommended for use in humans and animals. Below are the classifications with a list of products and their main therapeutic activity. D | Modulation by Promoting Periodontal Regeneration • Platelet-rich Plasma, Bone Morphogenic Proteins, Plasma-derived Growth Factor, Parathyroid Hormone, Enamel Matrix Derivatives E | Modulation of Nitric Oxide Synthase (Nos) Activity • Selective and Non Selective Nitric Oxide synthase inhibitors—nitic oxide inhibition F | Modulation of Cell Signaling Pathways in Periodontal Disease • P38 Mitogen-activated Protein Kinase (MAPK) Inhibitor—inhibition of MAPK G | Recombinant Anti- Inflammatory Cytokine • rhIL-11 – decreases cytokines H | Other Research is in the Developmental Stages on Host Modulation are • Aspirin-Triggered epimeril forms of DHA • Glucocorticoid-induced Annexin-A1 • Polgenolic zinc binding compounds • Semisynthetic glycosaminoglycan ether • Stem Cell therapy A | Modulation of the Immune Response Pro-inflammatory cytokine inhibition • Pentoxifylline, Certolizumab, Adalimumab, Golimumab – all block TNFalpha • Recombinant human – blocks IL-11 • Tocilizumab – blocks IL-6 • Ustekinumab – blocks IL-12, IL-23 • AIN 457 – blocks IL-17 Chemically modified tetracyclines (CMT) which inhibit Matrix Metalloproteinase (MMP) • Doxycycline – antibacterial and anti-collagenase properties • CMT1, CMT2, CMT3, CMT4, CMT5, CMT6, CMT7, CMT8 – no antibacterial but anti-collagenase properties B | Modulation of Arachidonic Acid Metabolites NSAIDS Indomethacin, Flurbiprofen, Ibuprofen, Naproxen, Meclofenamic Acid, Piroxicam – all block the cyclooxygenase pathways at different locations Pro Lipoxins, Resolvins and Protecting • 1-TDC ® which has lipoxins (stimulate the neutrophil to stop secreting pro-inflammatory cytokines), resolvins (stop neutrophil recruitment and infiltration), protectins and docosatrienes (both are pro-resolving lipoid mediators stabilizing the inflammatory response). 1-Tetradecanol Complex (1-TDC) is also an agonists that control the magnitude of inflammation • Omega 3 fatty acids, Eicospentanoic acid (EPA), Docosahexanoic Acid (DHA)—all are agonists that control the magnitude of inflammation C | Modulation of Bone Remodeling Inflammatory cytokine inhibition • Asprin, Ibuprofen, Ketorolac, Fluriprofen, Naproxen—all inhibit cyclooxygenase Bisphosphonates • Etidronate, Clodronate, Alendronate, Pamidronate, Risedronate, Ibandronate, Zoledronic Acid—all stop alveolar bone resorption • NOTE all have serious side affects and can not be recommended for periodontal treatment at this time • 1-Tetradecanol Complex (1-TDC) unknown pathway for bone remodeling at this time All these therapies with the exception of a few have potential side effects and limitations such as unknown durations, safety and uneconomical. Make sure to investigate the product thoroughly prior to any treatment. All these products need further research and long term trials.
